Pagination of medical records
Medical records are often voluminous and are not in any order whatsoever. We will sort records into each individual hospital and GP / medical practitioner. The records are subdivided into relevant sections and paginated accordingly. The notes are also placed in chronological order. This ensures that records are easily found. Each Volume of notes has an Index at the front. Please see "examples of work" for matters we have previously worked on.
We provide a detailed chronology of the records and this will be in accordance with instructions provided. We will detail appropriate treatment in respect of all notes or just in respect of a particular time (alleged negligence), depending upon instructions.

In clinical negligence cases, discrepancies in the records can be highlighted such as times of treatment and possible additional entries that have been made to the records at a later date.
Before instructing a medical expert it is essential to have all the relevant notes and records. We will notify you if any notes are missing.
All records should be sorted and returned within five working days and no later than ten working days. We will notify you, on receipt of instructions, if the timescale will be different.
